A 504 plan against an education programme
Both remove barriers for a learner with a disability. An individualised education programme changes the teaching itself and carries measurable goals; a 504 plan provides accommodations so that the ordinary programme is reachable, and carries none.

What it is for
A parent asks you which one their child has and what the difference is, and a wrong answer sends them to the wrong meeting. The difference is instruction against access, and you need to be able to say it in one sentence without looking it up.
How to think about it
Ask one question. Does this learner need the teaching itself changed, or do they need the barriers around the ordinary teaching taken away? The first is an individualised education programme and the second is a 504 plan.
